Key Responsibilities
Design and monitor outcomes dashboard and scorecards for assigned programs
Identify trends based on program results
Monitor and analyze program outcomes regularly
Recommend and implement action plans based on outcome data
Perform baseline and ongoing assessments to verify program requirements
Provide feedback to senior leadership on key performance outcomes
Address questions and concerns from leadership
Conduct presentations to program stakeholders to communicate data and program performance
Develop program components and support implementation of assigned programs
Educate provider agencies on best practices to achieve optimum outcomes
