OverviewDirect Meds is a fast-growing healthtech company transforming pharmacy and patient services at scale. Following strategic acquisitions and modernization efforts, we are rebuilding our infrastructure to support personalized, compliant, and efficient care delivery. Our current focus is architecting a modular AI agent framework that supports clinical workflows, billing automation, fulfillment coordination, and customer support; combining LLM orchestration with governed, hybrid AI-human workflows. This is a highly strategic and hands-on role responsible for translating the AI vision into reliable, compliant, and extensible technical systems. The position is fully remote.
Responsibilities- Architect and build the AI agent orchestration layer, including agent lifecycle management and modular agent workflow construction.
- Develop and implement agent logic, including prompt/LLM orchestration and decision-making workflows, that interact with data, routing, and integrations provided by the Senior Data & Cloud Engineer.
- Prototype, refine, and productionize LLM-based flows, ensuring safety, compliance, and consistency across patient-facing and internal workflows.
- Integrate agent flows with ActiveCampaign, Five9, Azure, internal APIs, and additional platforms in a cloud-agnostic manner.
- Implement guardrails for agent outputs, escalation conditions, and fallback logic to ensure workflow safety and seamless transitions between AI and human workflows.
- Collaborate boldly with the Director of AI and Senior Data & Cloud Engineer on solution blueprinting, pilots, and scaling deployments.
- Establish observability for agent behavior through logging, audit trails, and registry updates.
- Write, test, and deploy high-quality production code for agentic responsibilities.
Requirements/Technical Skills - Deep experience in backend engineering and orchestration frameworks (Celery, Temporal, Airflow, Kafka/EventBridge).
- Strong backend engineering experience with Laravel (preferred), PHP, or Python.
- Familiarity with TypeScript/JavaScript for integrations or dashboards is a plus.
- Strong SQL knowledge and comfort with rule engines/dynamic config (JSON/YAML).
- Experience designing and deploying LLM or agentic workflows (e.g., LangChain, LangGraph, AutoGen, custom orchestration).
- Integration experience with REST, GraphQL, webhooks, and message queues.
- Past implementation of logging, auditability, and compliance-driven backend services.
- Knowledge of Azure cloud services (preferred) and willingness to support a cloud-agnostic environment.
- Experience in healthcare, fintech, or other regulated environments strongly preferred.
- Fluency in English (written and spoken) is essential for effective collaboration and compliance communication.
- Collaborative work style in a fast-changing environment with cross-functional teams.
Architectural & Systems Thinking- Background in modular, scalable automation that leverages platform-level governance and normalized data.
- Ability to design agent workflows that extend across business units and meet compliance, rule, and security standards.

Compensation: $120k - 160k base + benefits, DOE. Flexible for the right candidate.
