Social Worker - Rehabilitation Case Manager

Hunter Brain Injury Service (HBIS)

About The Role

In this role you will have dual responsibilities. You may take on the role of either the Social Worker or the Case Manager for a client.

As a Social Worker you will be undertaking and providing trauma-informed psychosocial assessments with individuals and families, provide 1:1 counselling and support to clients and their families.

You will assist clients in navigating social systems including housing, Centrelink, and other services. Collaborating with your colleagues, you'll provide interdisciplinary care and support each other in managing complex psychosocial situations.

Your responsibilities will include attending case conference meetings, morning team huddles, and team meetings, as well as maintaining regular contact with insurance agencies (such as iCare, CTP, Workers Care, NDIS) and other service providers.

Where You’ll Be Working

  • The Hunter Brain Injury Service is 1 of 15 state-wide specialist rehabilitation services for adult clients who have experienced a traumatic brain injury.
  • The service located in Bar Beach, Newcastle provides a mixture of outpatient, outreach, and community residential care (5 overnight beds Monday to Friday only) within easy walking distance to the beach, shops, cafes and public transport.
  • The multidisciplinary team includes Physiotherapists, Occupational Therapists, Speech Pathologists, Social Workers, Neuropsychologists, Dietitians, Clinical Psychologist, Rehabilitation Assistants, Administration and Housekeeping staff. There is also a visiting Rehabilitation Medical Specialist and a Neuropsychiatrist.

  • In this role you will be supported by the Service Manager, Clinical Co-ordinator and other Social Workers who work in Brain Injury Units across NSW.
  • Access is provided to statewide resources and training to increase your skill and knowledge to support clients, families and staff.
  • Orientation will occur with the Service Manager and Clinical Coordinator. Time is allocated with each of the different disciplines and work side-by side with the other Social Worker.
  • We regularly celebrate birthdays, people going on maternity leave and host Social Club events such as meals out and trivia nights.
  • Our successful candidate will have regular catch-up meetings with the Service manager to review workloads and provide feedback. There are monthly in-services as well as other education opportunities through the NSW Brain Injury Directorate and the NSW Rehabilitation Network.

Requirements

Our successful candidate will possess the following:

  • Tertiary qualifications in Social Work or other qualification deemed equivalent by the employer, eligibility for membership of the Australian Association of Social Workers (AASW) and commitment to the AASW Code of Ethic.
  • Current eligibility to drive in NSW and the ability and willingness to travel for work purposes which may involve driving long distances.
